DMC Hub brings together educational tools and clinician resources maintained by David Coletta, PA-C. These terms explain their intended use.
Use the tools for their stated purpose
Patient tools help explain health topics and prepare questions for a visit. They do not diagnose you, choose a treatment, or replace your own clinician. Illustrations and interactive readouts are examples, not measurements of your body or predictions about your condition.
Clinician tools require professional judgment and any review described in the tool. A draft or demonstration is not a validated clinical protocol. Using DMC Hub does not create a clinician-patient relationship.
If you are having a medical emergency, contact emergency services rather than using this site.
Access and separate apps
The Hub can be browsed without an account or a Hub access fee. Some tools open separate websites, including inside the desk. Their own notices, terms, available features, and any payment requirements apply.
Links help you find resources. They do not mean that a tool is suitable for every person or that draft clinical content has been validated.
Your information
Data handling differs between tools. The Hub saves some preferences in your browser and contacts a weather provider when weather is enabled. Separate apps may send information to their own services when you use them.
Read the Privacy page and the notice in the particular tool before entering information. Leave names and other identifying details out of optional visit-preparation notes.
Content and attribution
You are welcome to link to DMC Hub and use its patient-education tools in conversations with your care team or patients. Please preserve educational framing and source attribution when sharing a handout.
Third-party libraries, fonts, illustrations, and source materials retain their own rights and license requirements. Do not present someone else’s work as your own. Check the relevant attribution or license before reproducing material beyond a tool’s provided sharing or printing features.

Tools and these terms may change. Check the date above and the information shown in the tool you are using. Saved or printed copies may describe an earlier version.
